Patient Advocacy Group Needs Help to Counter “t4” Only Therapy.


A patient advocacy group in the UK is fighting for recognition of combination therapy (t4 plus t3) to treat hypothyroidism (low thyroid levels). They are asking for people who have benefited from combination therapy to visit their website and demonstrate their support for this form of treatment. Why not have a look and decide for yourself?

Trouble with “t4 only” Therapy in Texas


Here is a question from our member nBarnard in Texas, about the failure to achieve an adequate clinical response to t4 treatment, and what to do about it. My response follows.

Hello Dr. Pepper,
I just found your blog and decided to join. I admire you for helping so many people this way. As with many people on this board, I have hypothyroidism. They removed my thyroid due to Hashimoto’s Thyroiditis a few years ago. It took a very long time before I started to feel better after taking the generic T4 meds. I was much heavier than I am now, since having started a special diet program where I lost 100+lbs. During my weight loss experience, I felt my best ever when I was down 60-80lbs. At that time, I was taking 250mcg Levothyroxine daily, with 25mcg every other day. (also taking maxzide 25mg, levonorgestre, and calcitriol – due to the parathyroid glands malfunctioning). Now I am down 100lbs and feel like I did before they removed my thyroid and I cannot lose any more weight. I am on a program where I should be losing everyday and nothing. My current dose is 200mcg per day. The doctor did blood work and they stated the levels were too high, so she lowered my thyroid medication. At the same time, I had to lower my blood pressure meds. My concern is that as the days go by, I feel worse and worse. My hypothyroid symptoms are so bad, that I cannot function. I am tired, cannot concentrate, dizzy, depressed, and on and on. I live in Texas and need a heater by my desk because I am so cold. I talked to the doctor about this and we did another blood test. She said the level dropped from 20 to 16 and that is an indication the med change is working. In the past when my numbers were too high, the doctor raised my medication. I am very concerned that this might be wrong dosage direction. I have appointments to see her, but cannot get in for a few months. In the mean, time, I am researching what can be done to help my situation. Any help would be appreciated.

As you know I can’t provide medical care or advice over the internet. I can make some general comments I hope you find useful.

First, congrats on losing all that weight! That is never an easy job. You mention your “levels” dropped from 20 to 16. Which levels: T4, or TSH ? That is a crucial piece of information.

At metabolism.com we have extensively reviewed the pro’s and con’s of using Armour Thyroid. In my medical practice I have found that hypothyroid symptoms that fail to respond to traditional t4 treatment often respond to Armour. It is my thought, shared by many, that the combination of thyroid hormones in Armour, particularly the presence of t3, is the reason Armour is superior to t4 only. Perhaps you can approach your physician about trying Armour, or alternatively adding some t3 (liothyronine) to t4.

S.A.D. stands for Standard American Diet – there was never a more apt acronym. The majority of U.S. citizens today subsist on processed fast food laden with refined carbohydrates and saturated fats. Give me five minutes and I’ll give you 1 very good reason for being vegetarian.

While fish is the most important dietary way to obtain the long-chain omega-3s eicosapentaenoic acid and docosahexaenoic acid, which has been shown to be essential in supporting brain health, low intake of eicosapentaenoic acid and docosahexaenoic acid in vegetarians does not adversely affect mood, reported by a new study (Nutr J. 2010;9:26. DOI:10.1186/1475-2891-9-26).

A research team from Arizona State University conducted a cross-sectional study to compare the mood of vegetarians who never eat fish with the mood of healthy omnivorous adults.

An overall total of 138 healthy Seventh Day Adventist adults residing in Arizona and California (64 vegetarians and 79 non-vegetarians) were enrolled in the study and completed a health history questionnaire, food frequency questionnaire and 2 psychometric tests, the Depression Anxiety Stress Scale and also the Profile of Mood States..

Vegetarians had significantly lower mean intakes of eicosapentaenoic acid, docosahexaenoic acid and the omega-6 arachidonic acid; they had higher intakes of the omega-3 alpha-linolenic acid and the omega-6 linoleic acid.

“Seed oils are the richest sources of α-linolenic acid, notably those of rapeseed (canola), soybeans, walnuts, flaxseed (Linseed oil), clary sage seeds, perilla, chia, and hemp.”

However, the vegetarians also reported significantly less negative emotion than omnivores in both psychometric tests. Mean total psychometric scores were positively linked to the mean intakes of eicosapentaenoic acid, docosahexaenoic acid and arachidonic acid , and inversely related to alpha-linolenic acid and linolenic acid intake.

The study team noted there is also the possibility that vegetarians may make better dietary choices and could generally be healthier and happier.

If you want to give it a try, here is an example of vegetarian recipe based on Italian cuisine

Italian Spaghetti with Zucchini

Ingredients:
* 17 oz. Spaghetti
* 24 oz. Of thin sliced zucchini
* 1 / 2 cup walnuts oil
* A few basil leaves
* 2 tablespoons of yeast flakes
* Salt and pepper

In a skillet or frying pan heat the oil and when hot, add garlic and zucchini. Raise heat and stir often to complete their cooking. They need to be golden and crispy outside and tender inside. Cook the pasta, drain and sauté in pan with zucchini, basil and yeast. Serve immediately.

Zucchini contain fewer calories and possess no fat. But they are a good source of potassium, e vitamin, ascorbic acid, folate, lutein and zeaxanthin.

These types of nutrients are extremely sensitive to heat and to enjoy their benefits you should find a quick solution to cook or even eat raw in salads.

From the therapeutic perspective, zucchini have laxative, refreshing, anti-inflammatory, diuretic and detoxifying action.

Dear Oprah, It was Fun While it Lasted!


It’s over between me and Oprah. If you are a regular reader of metabolism.com you probably know about my proposal to Oprah. Don’t get worked up, it wasn’t a marriage proposal. It was a proposal to create a Diabetes Lifestyles reality show for her new cable network. Here’s the background. Oprah is starting her “Own” network on cable and is hosting a contest for ideas to add to her line up. For the past 6 months visitors to Oprah’s website have been able to view all the ideas submitted by the public and vote for their favorite ones. My idea was to produce a Diabetes Lifestyle reality show called This Sweet Life. Over two months my idea acquired about 60 votes and was still in the running. The problem between Oprah and I started when she asked for a commitment. I’m not phobic about commitments but she asked for too much. In order to stay in the contest I had to commit to taking 6 weeks away from my medical practice to go to Los Angles to participate in filming of the conclusion of the contest. I’m sorry Oprah…I’m already committed to my medical practice so you can’t have me!

Maybe some other time. But it was fun while it lasted.
